• Intentional improper billing
• Unnecessary services
• Kickbacks
• Up coding
• Unbundling
• Falsification of health care provider credentials
• Falsification of provider financial solvency
• Related party contracting
• Incentives that limit services or referral
• Embezzlement and theft
• Billing Medicaid enrollees for RSN covered services
